Disaster Preparedness: A Guide for Chronic Dialysis Facilities – Second Edition - This CMS publication is a guide for dialysis facilities preparing for emergencies.

Preparing for Emergencies: A Guide for People on Dialysis - Each new dialysis patient receives a copy of this helpful booklet, which contains information on gathering and carrying important medical information, making alternative arrangements for treatment, and what diet to follow if dialysis must be delayed during an emergency. A Spanish version is also available. Published by the Centers for Medicare and Medicaid Services.

Planning for Natural Disasters and Other Types of Emergencies - This National Kidney Foundation publication is a guide for both dialysis facilities and patients preparing for emergencies.

Dialysis Patient Information Cards - These cards were designed with CMS oversight and approval to provide individual patient information on one side and toll-free numbers for large diaysis organizations and southeastern ESRD Networks on the other. Patients should carry this card in their wallet or purse to facilitate their care as a dialysis patient. You can complete and print these cards out in your unit. For the NW8 ID Number on the card, you can call Network 8 and ask for the number or leave it blank.
