Facility-specific Quality Assessment and Improvement Project - Optimizing Anemia Outcomes

A routine diagnosis for the ESRD patient, anemia impacts virtually all body systems. Common complications associated with anemia are shortness of breath, fatigue, impaired oxygen delivery to tissues, cardiac enlargement and impaired immune system response. Common causes of anemia include erythropoietin deficiency, blood loss, iron deficiency, inflammatory conditions and vitamin deficiencies. Though anemia can be effectively managed through the use of Erythropoiesis Stimulating Agents (ESAs), undesirably high hemoglobin levels may also occur.

Following the June 2011 FDA black-box warning and subsequent changes in the CMS Quality Incentive Payment, the July-December 2012 anemia project continues to focus on assisting facilities having > 10% of patients with hemoglobin greater than 12.0 gm/dL.
