Quality Improvement Reports

DFR / DFC Reports

The Dialysis Facility Reports (DFRs) and Quarterly Dialysis Facility Compare (QDFC) reports are created under contract to the Centers for Medicare & Medicaid Services (CMS) to provide dialysis facilities, patients, state surveyors, and regions with valuable information on patient characteristics, treatment patterns, hospitalization, mortality, and transplantation patterns in their facilities. 

The Quarterly Dialysis Facility Compare (QDFC) Reports provide Networks and Facilities with advance notice of new and updated quality measures for their facility prior to being displayed on Dialysis Facility Compare. These measures are updated quarterly in January, April, July, and October of each year. A timeline of upcoming preview periods can be found here. Note that these dates are subject to change.

Quality Incentive Program (QIP) Reports

The Centers for Medicare & Medicaid Services (CMS) administers the End-Stage Renal Disease (ESRD) Quality Incentive Program (QIP) to promote high-quality services in outpatient dialysis facilities treating patients with ESRD.  The first of its kind in Medicare, this program changes the way CMS pays for the treatment of patients with ESRD by linking a portion of payment directly to facilities’ performance on quality of care measures. These types of programs are known as “pay-for-performance” or “value-based purchasing” (VBP) programs.

The QIP Performance Score Report (PSR) includes facility information (CCN and contact information), achievement and improvement scores for each clinical measure, earned performance scores for all measures, Total Performance Score, unadjusted performance rates or ratios for clinical measures, performance standards applied, national average performance rates for clinical measures, payment reduction percentage, and text explaining the calculations used to generate the facility’s score.

The QIP Performance Score Certificate (PSC) includes achievement and improvement scores for each clinical measure, earned performance scores for all measures, Total Performance Score, unadjusted performance rates for clinical measures, performance standards applied, national median for clinical measures, and background information on the calculations and the QIP. Once the final PSC for the upcoming payment year is released, facilities must display their PSC in a prominent location within fifteen business days.
